General FAQs

  • To schedule an appointment at Birchwood Wellness, LLC you may call us at 515-289-1515. Our staff will assist you in scheduling and tell you what to expect at your first appointment. Please bring your current medications, your insurance card, and completed new patient paperwork to your first appointment.

    *Please be aware that due to full caseloads, we are not scheduling testing at this time.

FAQs Regarding Telehealth

  • Telehealth is providing services for your appointment via electronic technologies through smart phones/computers that is equipped with a web camera.

  • For our telehealth appointments we are using Zoom. Zoom is a HIPPA complaint virtual software system. To learn more about Zoom, click HERE.

  • Head over to our Telehealth page and click on your provider. Please only click if you have an appointment scheduled at that time.

  • No, you will need to follow the instructions above to make sure you get connected to the appropriate provider.

  • Yes, on our website please go to the ‘COVID-19’ tab. At the bottom of that page we have a button labeled ‘Telehealth Consent’. Follow the prompts to fill out the consent form.

  • In order to be eligible for telehealth appointments you will need to have access to a smartphone or computer that has audio and video as well as internet connection.

  • Most insurance companies cover mental health services provided via telehealth. As always, it is good practice to contact your insurance company prior to your appointment to confirm benefits. Patients are responsible for any fees insurance does not cover.

  • Yes, at the beginning of your session your provider will ask you to pay your copay and/or make a payment towards your balance.

  • Copays are required at the time of service as determined by your insurance plan.

  • No, this is a temporary service we are offering to our patients.

 FAQs Regarding Online Payments

  • No, we choose not to save card information for security purposes.

  • Yes, for privacy reasons we will need this form filled out anytime you would like to make an online payment.

  • Yes, after you complete the online payment form you will have the option to save a copy for your records. You may also email info@birchwoodwellness.com for a payment confirmation receipt.

  • You are more than welcome to mail in your payment or call our office and pay over the phone.

  • Yes, you are more than welcome to pay copays online.

  • No, you will need to call the office to get your current balance.

  • We would prefer one form per patient payment. However, if you do not have the time to complete two forms please specify to which accounts the payment should be applied.

  • Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises
